This is a community-wide survey (all residents of Calhoun & Liberty Counties)

Thank you for participating in the Calhoun and Liberty Community Health Survey! You are welcome to share this survey with all community members. This survey is for people who live, work, or spend time in Calhoun and Liberty Counties. Information gathered in the survey will help identify community health priorities and assist in developing health-related programs and services.

This survey will take 5-10 minutes to complete.

This survey is part of a Florida Department of Health in Calhoun County and Florida Department of Health in Liberty County initiative to:

1) Understand the health needs and concerns of people who live, work, or spend time in Calhoun and Liberty.

2) Identify the County’s strengths and challenges in providing a healthy environment for everyone.

3) Improve the health of the County and engage partners, organizations, and individuals in making the vision for a healthier Calhoun and Liberty Counties a reality.
